Data transitioning Manual Data entry

  • Patient’s electronic health information are recorded.
  • All the health-related details such as vital signs, medical problems, allergy, medications, prescriptions, lab/radiology orders, lab results, immunization, family health, socio psycho history, care plans and soap details of the patient are captured.
  • HL7 generation of files are carried out on Lab Orders and Immunization. The HL7 result files can also be read.
  • The SOAP notes added are converted into claims.
  • Patient summary can be viewed and generated in the CCD format.
  • The allergy, medication and e-Prescription are carried out using Rcopia (add-on subscription required)

Patients have access to EHR Patient Portal

  • Patients, who have a electronic health record in PracticeTrackerTM, will have an access to the Patient Portal.
  • Patients can access educational resources based on the diagnosis, lab results, and prescribed medications.
  • Patient details can be transferred to other Providers through general mail transmission (third party transfers) or DCDT mail transfers.

As you transition from one format to another, we help you get moving quickly by transferring your existing patient data into our system and implementing EHR.

  • Electronic to Electronic: Once you sign-up to PracticeTracker™, we offer a one-time service to import existing patient chart data into our software.
  • Paper to Electronic: If you have records in charts or on paper, all you have to do is scan and send us the documents or simply post them to our New Jersey office. We will capture your patient data and import it into the system.

Transitioning to PracticeTrackerTM EHR – 100% accuracy in transferring data

Our unique transition phase allows you to retain your paper-based systems, or EMR while moving to EHR. Data from scanned charts are imported into the PracticeTrackerTM EHR system. We also retain physical patient charts for continuation of existing processes while the transition continues.

Once you are comfortable, we customize the User Interface and transit to end-to-end Electronic Practice Management.
